Care coordination for 14 hospital sites, on one shared system

Context

A 14-site NHS trust whose referrals lived across four legacy systems. Clinicians were stitching together patient context by hand, and handover gaps were becoming a patient-safety issue.

Challenge

Centralize referral intake and patient context without a big-bang migration — and without a single lost referral during the swap.

What we built

We rebuilt referral intake as a phased platform: a data layer on top of the legacy systems, a new care-coordination UI, then a dual-run cutover that took nine months and never went down.
